Impact Testing/Physicals/Athletic Forms

For all student athletes who are interested in participating in Cross Country, forms are due July 18. All forms can be returned to the Athletic Office located in the main building. You can download forms by clicking on the link below.

Athletic Forms

If you still need to have a physical, the school doctor will be in the nurses office in the main building on July 17, 2014 from 3pm-6pm. a Parent or guardian must be present in order for the child to have a physical. 

For all Freshman, juniors, and those who haven't had an impact test two years, impact testing will be held on July 28th, 29th, Aug 4th, and 5th at 9am in Room 3 in the main building. This will be on a first come first serve basis.
